What To Bring To Your Consultation Appointment
- Insurance card
- Referral from your primary care physician, if necessary
- Physician referral forms if required by insurance
- A list of current prescribed and over-the-counter medications you are taking. Include dose and frequency
- Pertinent information about your medical and surgical history
- Any recent diagnostic tests or records you may have
What To Do For Your Sleep Study
- Do not consume any alcoholic beverages, caffeine (coffee, colas, tea, chocolate), or take sleeping pills for 24 hours prior to the test
- Please shower and wash your hair prior to the study
- Please do not use lotion, powders, perfume, cologne, after shave, conditioners, hair spray or gels in the hair
- Bring all your medications and take them as usual unless advised not to by the physician
- Please leave your valuables at home
- Do not nap the day of the sleep study
- Bring night wear and personal hygiene items to the sleep study
- Sleep log and questionnaire
Insurance and Billing
How you pay for your health care will depend on whether you have health insurance and the type of coverage you have. Please be aware that any costs not covered by insurance are your responsibility. We request payment at the time of your appointment for services that are not covered by insurance.
