Your CPAP machines will keep working just as before—you just need a new provider for the long term. 
Your prescriptions WILL run out and we cannot fill them, so find a new provider for the long term, see below.

Why is Doctor Bird’s office closing?
  1. Dr. Bird has been working in Tennessee, Louisiana, Texas, and other states for the last 2 years.  He lives in Salt Lake and has been commuting back and forth to cover his job and the clinic.  At this point he can no longer continue to perform sufficient clinic days to care for the patient load he has.
  2. Initially the nurse practitioner model was designed to assist but that did not work out.
 
What if I have an outstanding bill or amount owed:
  1. We have always contracted with a third-party company to do billing.  They will continue to bill in the standard fashion.  Please reach out to them or use our online portal to pay your bill.
 
Where can I go to get care?
  1. Insurance companies dictate which providers you can see.
  2. Primary care physicians are the best resource for which providers they trust in the community.
  3. We would recommend contacting your insurance company using the telephone number on the back of your health insurance card or using your online portal to determine sleep medicine providers in your area.
  4. We would then recommend you reach out to your primary care for a referral if needed.  Once you are established at a new office (ie. When you make an appointment, ask for their medical records fax number) then please read on this site about how to access your medical records.  Please note your primary care doctor also has copies of our notes and can forward those with the referral which is generally the easiest way.  We summarize a lot of data into our clinic notes and they will be very helpful to quickly get you established compared to the  hundreds of pages of unsorted data that often makes up a medical record.
  5. There are many new online resources as well: https://www.circlemedical.com/www.lofta.com and https://www.cpapnation.com/pages/telehealth-landing are a few of the new INEXPENSIVE ways to get CPAP care online–I cannot endorse nor provide recommendations on these websites I just know patients who have used them and found them to be helpful.  Please proceed with caution and use your own judgment.

What if I have an emergency?
  1. We are now closed. Please contact your primary care, call 911 or go to urgent care depending on the situation.
 
What if I need a refill?
  1. Unfortunately we will no longer be able to provide medication refills because we will no longer be able to see patients in person.  We would recommend establishing yourself with a new provider quickly and reaching out to your primary care to see if they can assist in getting you refills for a short period of time of established medications while you get a new sleep provider.
  2. Most controlled substances allow us to only provide 5 refills so please quickly reach out to providers and get established.
 
What if I just need to get a new prescription for my CPAP?
  1. www.lofta.com has been used by some of my clients.  Use your best judgment.

  3. Your primary care physician can often provide one.  This is a template you can fill out and give them.  Once signed you can give it to your durable medical equipment company like Medway, Lincare Apria etc. they will be able to fill your supplies.
Where are my medical records?
  1. We have sent copies to your primary care office every time you have been seen.  They have copies of our notes and any relevant studies.
  2. We provided copies to you when you came to see us of your sleep studies.
  3. We have a portal (it is in the process of being updated but if you sign up now by the beginning of 2026 you will have full access—so you can see and download your records in a secure format yourself!  https://www.patientfusion.com/  Look for an email from “Patient Fusion” (It may have been a LONG time ago) and make sure you know your date of birth and the phone number you gave at the office. ALL patients are invited to the portal.
